Very good, one major problem so far (4 stars)
As others have described, the Journal transforms well into an iPad app and will presumably do better in subsequent updates. One unforgivable error is that several of the illustrations won't open because they require Adobe Flash Player to be installed. Why would an app specifically designed for the iPad require Flash Player when all know this is not a feature available on the iPad?

Lock Rotation (4 stars)
Excellent app; allows offline reading without having to manually download individual PDF files. The orientation, however must not be changed while reading or the app takes you back to the beginning of the article. Also the bookmark feature marks the article and not your place within the article. Videos do not play as they require Adobe flash (HTML 5 may have been better) perhaps a link to the web video could be a temporary fix for individual

More Than A Little Buggy (2 stars)
This reproduces the NEJM print experience, but shouldn't we have gotten more from the medical journal with the most resources to spend on a state-of-the-art app? Add to that you can't go deeper than the TOC because the app crashes and you get a promising glimpse of a very disappointing product.
